Spontaneous carotid - cavernous fistula produced by ruptured aneurysm of the meningohypophyseal branch of the internal carotid artery

Abstract: ✓ The authors report a case in which the cause of a “spontaneous” carotid-cavernous fistula could not be demonstrated by selective angiography of external and internal carotid arteries. However, postmortem study of the cavernous sinuses revealed an unsuspected ruptured aneurysm of the internal carotid artery at the origin of the meningohypophyseal branch.

“…Unusual cases, such as traumatic aneurysm of the supraclinoid ICA in association with CCF 8, trigeminal cavernous fistulae 8 , CCF produced by ruptured aneurysm of the meningohypophyseal branch and post-traumatic fistula of the PCoA aneurysm-cavernous sinus and the PCoA-cavernous sinus have been reported [1][2][3]9 .…”

“…Barrow established that traumatic CCSFs are usually type A direct fistulas, either formed by a tear in the cavernous portion of the ICA resulting in a high-pressure high-flow shunt, or formed in consequence of the spontaneous rupture of an intracavernous aneurysm into the CS [9,25,36]. All the other three types are indirect low-flow shunts.…”

“…86 Management then is dependent on location and symptomatic presentation. Obrador et al 87 citing observations made by Jeanamart note that aneurysms of the internal carotid artery within the cavernous sinus represent approximately 3-11% of the total number of intracranial aneurysms. These authors go on to state that many larger aneurysms within the cavernous sinus structure, because of their obliterative nature, often times act as expanding extradural or paraclinoid masses.…”
