Spontaneous hypoglycemia in patients with cystic fibrosis

Abstract: Objective: Diabetes frequently complicates cystic fibrosis (CF) without fasting hyperglycemia or despite spontaneous hypoglycemia (anecdotally ascribed to malnutrition), whose prevalence, clinical meaning, and relationship with glucose tolerance and clinical/nutritional status were not previously investigated. The relationship of CF genotype with insulin secretion control is also unclear. Design and methods: A total of 129 CF patients without stable diabetes received 188 oral glucose tolerance tests. Distribut… Show more

“…Although there are reports on hypoglycaemic episodes in patients with CF [15] the role of exercise as trigger of symptomatic hypoglycaemias has not been explored. We found an incidence of exercise-induced symptomatic hypoglycaemia in 0.4% of all patients represented in our caregiver survey and 0.8% in the patient survey.…”

“…Consistent with this, insulin levels failed to decrease with lower glucose levels in nursing CF kits, while glucose and insulin levels were positively correlated in nursing non-CF kits (Supplemental Figure 3A). These tendencies observed in CF kits toward insulin oversecretion might play a role in the hypoglycemia that is observed in humans with CF (63,64).…”

“…Fasting glycemia is generally lower than in other forms of diabetes and fasting hypoglycemia has been reported (14). Therefore, the categories of 'diabetes without fasting hyperglycemia' (15) and of 'indeterminate glucose tolerance' (isolated hyperglycemia at mid-oral glucose tolerance test (OGTT) or continuous glucose monitoring) were established (16).…”
