Spontaneous osteonecrosis of the maxilla

Abstract: In the past decade there has been a rise of osteonecrosis of the jaws (ONJ) predominately related to bisphosphonate therapy and osteoradionecrosis secondary to radiotherapy. However, osteonecrosis can occur in the absence of these agents. A case of idiopathic osteonecrosis of the maxilla is reported and the literature discussed. Clinical relevance: It is not always possible to make a definitive diagnosis.

“…Possible etiologic factors for exposed bone lesions were reported in the literature and consist of trauma, odontogenic infections, herpes zoster infection associated osteonecrosis, HIV-associated necrotizing ulcerative periodontitis, benign sequestration of the lingual plate [1], long-term corticosteroid usage [4], ischaemia, occlusion, and coagulopathies [5,6,7].…”

“…The causes can be divided into traumatic, non-traumatic, and idiopathic (spontaneous). Traumatic osteonecrosis is caused by thermal, mechanical, or chemical damage, while non-traumatic osteonecrosis is caused by neoplasms, infection, acquired and congenital diseases, and the use of narcotics [4]. Rare cases of idiopathic osteonecrosis have also been described [4,5].…”

“…Non-traumatic osteonecrosis is associated with infections, neoplasms, use of narcotics, and vascular causes such as ischemia, occlusion, coagulopathy, hemoglobinopathy, and some autoimmune diseases. However, some cases of idiopathic osteonecrosis that developed without an obvious etiological cause have also been described [4,5].…”
