Spontaneous Perirenal Hemorrhage after Rupture of an Aneurysm in Case of Polyarteritis Nodosa along with Anuric Renal Failure

Abstract: We report a case of massive perirenal hemorrhage owing to an inferior seg-mental arterial rupture of an aneurysm, alongside a primarily inapparent polyarteritis nodosa associated with hepatitis B and C. We come to speak of the diagnostic procedure such as angiography, computerized tomography and MRI as well as the intervening measures like catheter embolization and surgical revision.

“…Visceral aneurysms in PAN are formed, in general, due to the effects of the vasculitis, [18][19][20] whereas only one case of histologically-proven intracranial aneurysm formed by necrotizing arteritis has been reported. 22) In the present case, no pathological findings specific for PAN, such as fibrinoid degeneration, were confirmed.…”

“…Aneurysm formation in PN is commonly thought to be due to the vasculitis itself 20–22 . The present case, however, suggested a new concept: that the cause of aneurysm in PN involves SAM.…”

“…Aneurysm formation in PN is commonly thought to be due to the vasculitis itself. [20][21][22] The present case, however, suggested a new concept: that the cause of aneurysm in PN involves SAM. This suggests that it may be clinically important to follow up patients with PN because PN and SAM could develop by different mechanisms and steroid therapy may not be beneficial for SAM itself.…”

“…PAN is 3 times more common in males, generally aged 40–60 years, and in 30% of the cases hepatitis B is involved. Only 10 cases of bilateral subcapsular or perirenal hematomas secondary to PAN have been reported in the literature [1, 2, 3]. The lesions lead to glomerular focal nephritis and hypertension, and in more severe cases patients may develop aneurysms that can rupture and create a sudden dramatic progression [4].…”
