Spontaneous Regression of Bilateral Conjunctival Extranodal Marginal Zone B-cell Lymphoma of Mucosa-Associated Lymphoid Tissue

Abstract: We report the case of a patient who showed spontaneous regression of bilateral conjunctival extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma). A 72-year-old man underwent excisional biopsy for salmon-pink lesions involving the whole circumference of the conjunctiva in the right eye and the lower fornix in the left eye. Histopathology and immunohistochemistry showed MALT lymphoma with immunoglobulin kappa monotype shared by the lesions in both eyes. Because the patien… Show more

“…The conjunctival lesions (salmon-pink lesions) in the present patient appeared clinically similar to B-cell and Tcell lymphomas [1][2][3][4]6,7 as well as benign lymphoid hyperplasia. [8][9][10] Leukemic infiltration of the conjunctiva is rare but does occur to present the same clinical picture as lymphoma.…”

“…The most frequent pathological type of primary lymphoma, occurring in the conjunctiva, is extranodal marginal zone Bcell lymphoma of the mucosa-associated lymphoid tissue (MALT), or MALT lymphoma. [1][2][3][4] Indeed, the conjunctiva has been shown to have mucosa (conjunctiva)-associated lymphoid tissue, which can be acquired in the normal condition. 5 Other less common types of primary lymphoma in the conjunctiva, which have been reported so far, include T-cell lymphoma 6 and Burkitt lymphoma.…”

“…58 Thus, EMZL appears to develop as a result of prolonged antigen stimulation that leads to loss of regulation of B-lymphocyte proliferation and differentiation. 37 Several characteristics of conjunctival EMZL give rise to the suspicion that this mechanism also triggers EMZL development in the conjunctiva: 1) conjunctival EMZL is often bilateral and may show spontaneous remission, which could be the result of bilateral conjunctival infection and clearance of an antigen, respectively 84,85 ; 2) the t(11;18)(q21;q21) translocation, which is related to oxidative damage induced by genotoxic factors such as H pylori or C psittaci infection, is present in 19% of conjunctival EMZLs 148 ; 3) in some cases, patients with conjunctival EMZL have a history of preceding infectious conjunctivitis. 42 A number of studies have investigated the association between the infectious agents and conjunctival lymphoma.…”
