Spontaneous Rupture of Hepatocellular Carcinoma and Vascular Injury

Abstract: Because spontaneous rupture of hepatocellular carcinoma (HCC) is one kind of bleeding complication related to the blood vessels, the possible mechanism of this rupture should occur on the blood vessel itself. Our hypothesis, which has not yet been investigated, is that the vascular integrity of HCC might be damaged during vascular injury. Design: We examined semiquantitatively the expression of von Willebrand factor, elastin, neutrophil elastase, type IV collagen, and collagenase in 23 specimens of HCC with sp… Show more

“…Spontaneous tumor rupture, a life-threatening situation for patients with large HCC, is also a known adverse prognostic factor after hepatic resection 7 . Spontaneous tumor rupture might be caused by the rapid expansion of the tumor, but the molecular mechanism of spontaneous tumor rupture is still not fully understood 21 . Transhepatic arterial embolization followed by hepatic resection has been proposed to be a rational treatment for the majority HCC patients with tumor rupture if the tumor could be resected 22 …”

“…Hızlı tümör büyümesi nedeniyle oluşan santral nekroz, trombositopeni ve protrombin sentez bozukluğuyla ilişkili koagülopati, direkt tümör invazyonu nedeniyle ortaya çıkan venöz hipertansiyon, travma ve solunum hareketleriyle diafragmın yarattığı kompresyon literatürde spontan HSK rüptürü ile ilişkili olduğu düşünülen çeşitli mekanizmalar arasındadır [7,8]. HSK rüptürü tanısını koymak özellikle öncesinde siroz veya HSK tanısı olmayan hastalarda zordur [9].…”

“…Patients with FLC treated with surgical resection have a more favorable outcome than nonfibrolamellar hepatocellular carcinomas with reported 5-year survival ranging from 58% to 82% [1]. Spontaneous rupture of HCC occurs in 3% to 26% of all patients with HCC, and high mortality rate, in the range of 32% to 67%, has been reported [3, 4] The pathogenesis of liver rupture is not well known; several hypotheses have been proposed in the literature, including a fast-growing tumor resulting in central necrosis, coagulopathy, fragility, vascular congestion and venous compression, or infiltration of the tumor [5]. Tumor size >5 cm and extrahepatic invasion have been associated with high propensity to rupture [6–8].…”
