Acid-fast bacteria can be implicated in skin and soft tissue infections. Diagnostic identification can be challenging or not feasible by routine laboratory techniques, especially if there is no access to the Matrix Assisted Laser Desorption Ionization Time of Flight Mass Spectrometry (MALDI-TOF MS) technology. Here, we present two cases of skin and soft tissue infections caused by two different acid-fast bacteria, Nocardia brasiliensis and Mycobacterium marinum . They both grew on Löwenstein–Jensen medium, Sabouraud agar medium and blood agar medium. Both bacteria appeared acid-fast by Ziehl–Neelsen stain and Gram-positive by Gram stain. The identification was performed by MALDI-TOF MS and gene analysis. N. brasiliensis and nontuberculous mycobacterium M. marinum represent rare pathogens that cause severe skin and soft tissue infections. Failure to identify the causative agent and subsequent inappropriate or inadequate treatment may lead to severe complications or even disseminated disease, especially in immunocompromised individuals.

“…The minimum inhibitory concentrations (MICs) of levofloxacin, ciprofloxacin, and quinolones about M. marinum are high while keeping lower for rifampin, moxifloxacin, ethambutol, clarithromycin, linezolid, and tetracyclines. The results (Koushk-Jalali et al, 2019;Yeo et al, 2019;Castillo et al, 2020;Strobel et al, 2022) show that rifampicin, clarithromycin, sulfonamides, doxycycline, minocycline, and ethambutol are more suitable choices. Non-tuberculosis mycobacteria (NTM) skin infections have become increasingly prevalent in recent years, presenting a unique challenge in clinical management. This review explored the complexities of NTM infections localized to the superficial tissues and provided valuable insights into the optimal therapeutic strategies. The antibiotic selection should base on NTM species and their susceptibility profiles. It is recommended to adopt a comprehensive approach that considers the unique characteristics of superficial tissues to improve treatment effectiveness and reduce the incidence of adverse reactions, infection recurrence, and treatment failure. Infection control measures, patient education, and close monitoring should complement the treatment strategies to achieve favorable outcomes in managing NTM skin infections. Further efforts are warranted to elucidate factors and mechanisms contributing to treatment resistance and relapse. Future research should focus on exploring novel treatment options, innovative drug development/delivery platforms, and precise methodologies for determining therapeutic duration. Longitudinal studies are also needed to assess the long-term safety profiles of the integrated approaches.

Difficulties in the diagnosis of fish tank granuloma – case report

Introduction: Fish tank granuloma is a rare dermatitis caused by Mycobacterium marinum. Infection occurs through contact of damaged skin with water or aquatic animals infected with these bacteria. Aim: It is likely that skin infection caused by M. marinum transmitted from Clarias gariepinus has as yet not been reported in literature. Our case report is presumably the first publication. Dermatoscopy as a useful method in the diagnosis of skin infections caused by atypical mycobacteria. Our article presents new dermatoscopic features of fish tank granuloma. Case study: The case report applies to a 30-year old, white, Caucasian man referred because of a single, well-demarcated plaque measuring 2 × 4 cm, localized on third finger on left hand above proximal phalanx, without subjective symptoms. The diagnosis was confirmed by medical history, dermoscopy and histopathological examination. Patient was successfully treated with sulfamethoxazole and trimethoprim. Results and discussion: Due to the fact that M. marinum infection is uncommon, in case of clinical suspicion it is necessary to perform skin biopsy, culture and tissue PCR analysis. Knowledge about the opportunities and limitations of theses laboratory tests is pivotal to reasonable clinical decision-making. Conclusions: Dermoscopy is useful to make a diagnosis, but there are still too few accounts in literature. The correct diagnosis determines the effective treatment.
