Square-wave jerks and square-wave oscillations: treatment with valproic acid

Abstract: Experimental studies in monkeys have demonstrated that injection of gamma-aminobutyric acid (GABA) agonists into the superior colliculus restricts saccadic eye movements, while injection of GABA antagonists leads to square-wave jerks (SWJ). To investigate the role of GABA in saccadic interruptions of fixation, we treated five patients who had SWJ and square-wave oscillations (SWO) with valproic acid (VPA) (2000 mg/d), which has been shown to increase brain GABA levels. VPA effectively reduced the SWJ/SWO in th… Show more

“…In addition to therapy for any underlying process such as tumour or encephalitis, treatment with immunoglobulins or prednisolone may be occasionally effective [70]. Four of five patients with square‐wave oscillations, probably a related fixation disturbance, showed an improvement on therapy with valproic acid [71] or in patients with hereditary spinocerebellar atxia on therapy with memantine 20 mg/daily [72]. In single cases, an improvement has been observed during treatment with propranolol (40–80 mg p.o.…”
