Staged Surgical Treatment of Primary Aortoesophageal Fistula

Abstract: Aortoesophageal fistula (AEF) is a rare and potentially fatal disease that causes massive gastrointestinal bleeding. Therefore, early diagnosis and treatment are essential to prevent mortality. Controlling the massive bleeding is the most important aspect of treating AEF. The traditional surgical treatment was emergent thoracotomy, but intraoperative or perioperative mortality was high. We report a case of a patient presenting with hematemesis who was successfully treated by a staged treatment, in which bridgi… Show more

“…9 Traditional surgical management consists of open thoracotomy with repair of the aortic lesion and esophagus. 10 However, open surgical repair is associated with significant morbidity and mortality. 10 Endovascular aortic repair through stent-graft placement offers a less invasive option for bleeding control that is particularly useful for patients who are not candidates for open repair.…”

“…Because AEF is not a common cause of upper gastrointestinal hemorrhage, early diagnosis may be difficult ( Hwang & Cho, 2019 ). Although EGD is useful for the diagnosis of AEFs in patients with gastrointestinal hemorrhage, its diagnosis rate is low at around 25% ( Kayashima et al, 2019 ).…”

“…Although EGD is useful for the diagnosis of AEFs in patients with gastrointestinal hemorrhage, its diagnosis rate is low at around 25% ( Kayashima et al, 2019 ). Also, it should be noted that EGD can irritate blood clots, leading to severe re-hemorrhage ( Hwang & Cho, 2019 ). The characteristic finding of AEF on EGD is a submucosal tumor-like protrusion, ulcerative lesion, or pulsating protrusion with central fistula ( Yang et al, 2018 ).…”

“…So antibiotic-soaked prosthetic grafts covered with omental flaps would be an effective alternative, especially when most of the AEF required emergency operations. Esophageal repair of the fistula was another option, especially when esophageal defects were minimal without gross contamination [ 32 , 33 ].…”
