Objectives: To investigate the safety, efficacy, and practicability of minimally invasive percutaneous nephrolithotomy (MPCNL) with the aid of a patented irrigation clearance system in treating renal staghorn calculi. Methods: From August 2009 to July 2014, 4 hospitals had executed a prospective multicenter study with a total of 912 cases. The patients were randomly divided into 3 groups: suctioning MPCNL, standard percutaneous nephrolithotomy (PCNL), and traditional MPCNL groups. Multiple operative and perioperative parameters were compared. Results: Blood loss and intrapelvic pressure in the suctioning MPCNL group were significantly less than those in the standard PCNL group. The average operation time, intrapelvic pressure, and amount of bleeding in the suctioning MPCNL group were better than those in the traditional MPCNL group. The suctioning MPCNL used one tract more frequently and 2 or 3 tracts less frequently than the standard MPCNL and traditional MPCNL groups. The stone-free rate by one surgery in the suctioning MPCNL group was significantly higher than that in standard PCNL and traditional MPNCL groups. Conclusions: Suctioning MPCNL using our patented system shows several advantages in treating renal staghorn calculi, including minimal invasion, shorter operation time, lower intrapelvic pressure, less bleeding and the need for a smaller number of percutaneous tracts, and higher stone clearance rate by one surgery.

Purpose: To assess bleeding complications in totally tubeless double-access percutaneous nephrolithotomy (PCNL) and compare it with the single-access method. Methods: One hundred and seven patients with large or scattered renal stones were enrolled in this cohort study. Totally tubeless PCNL with one access (group A, 70 cases) or two accesses (group B, 37 cases) was done and bleeding was assessed by measuring the hemoglobin drop of the patients, blood transfusion rate and need for performing angioembolization. Results: The mean (SD) hemoglobin drops in groups A and B were 1.97 (1.24) and 2.31 (1.24), respectively; p = 0.176. Blood transfusion rates in two groups were 7.1 and 10.8% (p = 0.716), respectively. None of our patients required angioembolization. The average hemoglobin drop and blood transfusion rate with double tracts were approximately near to those in single tract group and in acceptable ranges. The average hospital stay in groups A and B were 3.1 and 3.2 days (p = 0.074), respectively. There was no leakage from puncture site/s and stone free rates in the two groups were 85.7 and 97.3% (p = 0.093). Conclusion: Totally tubeless double-access PCNL is feasible and bleeding complications are comparable to single access totally tubeless PCNL making it an effective strategy to achieve more stone free rates.

GirişS truvit (magnezyum amonyum fosfat) taşları, üreaz pozitif bakterilerin neden olduğu idrar yolu enfeksiyonları sonucunda gelişmektedir. Bu sebepten ötürü; struvit taşları enfeksiyon taşı olarak da anılmaktadır. Bütün üriner sistem taşlarının %10-15'ini oluşturan struvit taşları daha ziyade kadınlarda (kadın/erkek: 2/1) tespit edilmektedir. Cinsiyetler arasındaki bu farklılık, üriner sistem enfeksiyonlarının genel olarak kadınlarda daha sık görülmesi ile ilişkilendirilmiştir. Her iki cinsiyette de, enfeksiyon taşı insidansı yıllar içerisin-de azalma eğilimi göstermektedir (1,2).Bu taş tipi, staghorn kalküllerin %25'ini oluşturmaktadır (3). Enfeksiyon taşlarının, diğer taş tiplerinin aksine, renal kolik ile farkedilme ihtimali düşüktür. Başvuru anında en sık görülen yakınmalar; yan ağrısı ve/veya karın ağrısı (%70), yüksek ateş (%26) ve makroskopik hematüri (%18) olarak kaydedilmiştir. Hastaların %8'i asemptomatik kalabilirken, %1'i sepsis ya da tekrarlayan üriner sistem enfeksiyonları nedeni ile başvururlar (4,5). Kadın cinsiyet, konjenital üriner sistem malformasyonları, üriner obstrüksiyona bağlı üriner staz, üriner diversiyon, nörojenik mesane, kalıcı foley sonda mevcudiyeti, distal renal tübüler asidoz, medüller sünger böbrek ve diyabetes mellitus enfeksiyon taşı oluşum riskini arttıran faktörler olarak bilinmektedir (6).Strüvit taşları böbrekte veya mesanede oluşabilir. Böbrekte yerleşim gösteren enfeksiyon taşlarının %15'i bilateraldir (4). Enfeksiyon taşlarının oluşumu ortalama 4-6 hafta sürerken, bu süre kalsiyum taşları için 13 haftadan uzun olarak bilinmektedir (7,8). Enfeksiyon taşlarının kompozisyonu genellikle heterojen olup, bünyesinde değişik oranlarda magnezyum amonyum fosfat, monoamonyum ürat ve karbonat apatit barındırabilir (8). Bu derlemede üriner sistem enfeksiyon taşlarının patogenezi ve tedavi yaklaşımları irdelenecektir. Patogenez ve MikrobiyolojiHem gram (+) hem de gram (-) bakteriler üreaz enzimi üre-tebilir. Proteus, stafilokokkus, psödomonas, providencia, klebsiella ve morganella struvit taşları ile en sık ilişkilendirilmiş bakteri tipleridir. Bu bakteri tiplerinin her alt türü aynı oranda üreaz enzimi üretmeyebilir. Komplike idrar yolu enfeksiyonları ile ilgili bir çalışmada; Klebsiella ve stafilokokların üreaz üretme oranı sırasıyla, %84 ve %55'i iken; aynı oran proteus, providencia ve morganella morganii için %100 ölçülmüştür. Toplumsal kökenli idrar yolu enfeksiyonlarının en sık sebebi olan E.coli'nin üreaz üretme ihtimali %1.4 düzeyindedir (8). Dolayısıyla struvit taşları ile E.coli arasındaki ilişki oldukça zayıftır. Bu bulgu, üreaz üreten bakterilerin tespit edilebilmesi için struvit taşı olan hastalarda hem idrarın hem de taşın mikrobiyolojik olarak incelenmesi gerektiğini desteklemektedir. Ayrıca, struvit taşı olan bireylerin idrar ve taş kültüründe tespit edilen bakteriler birbirinden farklı olabilir.Enfeksiyon taşlarının oluşabilmesi için; üre, su, kalsiyum, magnezyum, fosfat, idrar pH'sının 6.8-7.2'den fazla olması ve üreaz enzimi gereklidir.
