Standardized and Individualized Parenteral Nutrition Mixtures in a Pediatric Home Parenteral Nutrition Population

Abstract: Objectives: Studies evaluating efficacy or safety of standardized parenteral nutrition (PN) versus individualized PN are lacking. We aimed to assess effects on growth and safety of standardized PN compared with individualized PN in our Home PN group. Methods: Descriptive cohort study in Dutch children on Home PN, in which standardized PN was compared with individualized PN. Both groups received similar micronutrient-supplementation. Primary outcome was growth over 2 years, secondary outcomes were electrolyte d… Show more

“…The clinical feature and the outcome of both adults and children with CIF have been reported by several individual center and country surveys, which described factors associated with the probability of intestinal rehabilitation as well as with the risk of CIF/HPN-related major complications and death [10,[12][13][14][15][16][17][18][19][20].…”

“…There is little evidence regarding optimal weaning strategies, and the potential for standardized solutions suitable for HPN is relatively underexplored. There is emerging evidence that the use of standardized home PN solutions may be comparable to individualized solutions in terms of maintaining growth and serum biochemistry, though larger studies are needed to confirm this 27 . The use of strategies to reduce complications such as infection and IFALD have not been researched thoroughly in the context of HPN.…”

“…While individualized PN bags contain all the necessary nutritional supplies for the patient, they have the disadvantage of having a limited stability of four to six days, in contrast with standard commercial PN bags that are usually stable at room temperature as long as 24 months. The longer shelf life and increased availability of commercial PN solutions could therefore help reduce the frequency of PN bag deliveries (5,7,15). Additionally, it seems to decrease hospital pharmacies' workload and costs (7,14,16).…”
