Statin tedavisinde hasta uyumu: Medya ne kadar etkili?

Abstract

Amaç: Son dönemlerde görsel ve yazılı medya aracılığıyla statin grubu ilaç kullanımının gereksiz olduğuna ya da yan etkilerinin bulunduğuna yönelik tartışmalar yaşanmaktadır. Çalışmamızda, güncel kılavuzların önerdiği biçimde statin tedavisi kullanma endikasyonu bulunan hastalarda, tedavi uyumuna medyanın etkisinin araştırılması amaçlandı.Yöntem: Statin tedavisi kullanmakta olan hastalar, tedaviyi bırakmış olan hastalar ve aile hekimliği polikliniğinde ilk kez dislipidemi tanısı konulup statin tedavisi endikasyonu olan hastalar çalışma grubunu oluşturdu. Çalışmaya 286 hasta dahil edildi. Hastaların demografik ve klinik özellikleri, kardiyovasküler hastalık risk faktörleri, başvuru esnasındaki lipid düzeyleri belirlendi. Çalışma Avrupa Kardiyoloji Derneği 2011 Dislipidemi Tedavi Kılavuzunun önerileri doğrultusunda yürütüldü.Bulgular: Hastaların %76,2’sine (s=218/286) daha önceden dislipidemi tanısı ile statin tedavisi başlandığı saptandı. Daha önce statin başlanan hastaların %54,1’i (s=118/218) son beş yıl içerisinde ilaç kullanmayı bırakmıştı. Lipid düşürücü ilaç kullanmaya devam eden hastaların sadece %36,6’sının hedef LDL değerlerine ulaştığı tespit edildi. Daha önce başlanmış olan statin tedavisini bırakan hastaların %52,9’u tedaviyi, statinler ile ilgili olarak medyada yer alan olumsuz haberlerden etkilendikleri için bırakmıştı. Endikasyonu olanlara önerilen lipid düşürücü ilaç tedavisini reddeden hastaların %93,8’inin yine medyada çıkan haberleri gerekçe gösterdiği gözlendi.Sonuç: Bu çalışma statin kullanan ve kullanması gereken hastaların tedaviye devamlılığını ve uyumunu göstermesi bakımından önemlidir. Çalışmamız, daha önce reçete edilen ancak tedaviyi bırakan ve tedavi başlanmasını kabul etmeyen hastaların çok önemli bir kısmında yazılı ya da görsel medyanın olumsuz etkilerinin bulunduğunu göstermiştir.
