Status epilepticus and seizures induced by iopamidol myelography

Abstract: Iopamidol myelography (especially if cervical) is associated with a risk of seizures in non-epileptic individuals and can induce SE in patients with epilepsy. Patients should be informed about the risk of seizure induction.

“…Since urinary Na or serum ADH concentrations were not measured, we are left to speculate the administration of the contrast agent that perhaps resulted in an increase in urinary Na excretion. There have been a few cases of neurological complications after administration of contrast agents but most of these cases reflect neurological deterioration from agents directly injected into the epidural or subarachnoid space in myelography [7][8][9]. The case most similar to ours involves a report of acute encephalopathy in a child during heart catheterization where the contrast agent (Optiray, as in this case) was detected within the brain [10].…”

“…Although CT myelography is the established standard in adults 13,18,19,26-28 and infants, 12,14,15,29 the risk of infection and seizure related to intrathecal contrast administration [20][21][22] and evidence that early exposure to radiation may increase later risks of malignancy 23,24 make identifying an alternative important.…”

“…14 CT myelography requires a lumbar puncture for injection of intrathecal contrast, with attendant risks of infection and seizure. [20][21][22] Recent studies have also raised concern for malignancy with early exposure of children to radiation. 23,24 MR myelography can be performed without injection of contrast and is a promising alternative.…”

“…Despite their low neurotoxic effect, seizures can occur, as described by some authors. [66][67][68][69] Incidence appears to be higher in procedures in the cervical area. 67 Whenever CAs are used, the neurotoxic effect (possibility of seizures), even with CAs approved for intrathecal use, must be taken into account, and benzodiazepines should be at hand.…”
