Purpose To investigate the medium-term clinical outcomes and risk factors for primary graft failure after Descemet membrane endothelial keratoplasty (DMEK) in Thai patients. Patients and Methods This is a single-center retrospective cohort study. Sixty-two eyes of 62 patients who underwent DMEK at Ramathibodi Hospital, Bangkok, Thailand, with a minimum of 24-month follow-up were recruited. Preoperative donor and recipient characteristics, intraoperative data, and postoperative outcomes including best-corrected visual acuity (BCVA), graft clarity, endothelial cell density (ECD), central corneal thickness (CCT), and complications were evaluated at 1, 3, 6, 12, and 24 months after surgery. Results The mean age of the patients was 67.2 ± 9.9 years, and 52% were female. The mean follow-up time was 37.5 ± 11.0 months. The most common indications for DMEK were Fuchs’ endothelial corneal dystrophy (FECD) (53.2%) and pseudophakic bullous keratopathy (PBK) (17.7%). Nearly half of patients had triple-DMEK. The median preoperative BCVA was 20/400. Postoperative BCVA of ≥ 20/40 was reached in 37.1% and of ≥ 20/20 in 6.5% after 1 month which increased to 54.8% and 17.7% after 3 months; and to 67.7% and 27.4% after 24 months. Endothelial cell loss (ECL) at 3, 6, 12, and 24 months was 30.5%, 33.8%, 44.4%, and 45.9%, respectively. Graft diameter was the single factor, showing a significant relationship with postoperative ECD. Most frequent postoperative complications included graft detachment (22.6%), increased IOP/glaucoma (17.7%), and primary graft failure (16.1%). In univariate analysis, death-to-operation time and the diagnosis of PBK were significantly associated with the occurrence of primary graft failure. Conclusion DMEK is a safe, effective, and feasible treatment for endothelial failure in Asian eyes. Careful case selection, use of relatively fresh donor tissues, and appropriate surgical techniques can prevent primary graft failure and facilitate optimal outcomes following surgery.

Reverse 4-Throw Pupilloplasty for Endothelial Keratoplasty

Purpose: The aim of this study was to describe the feasibility and applicability of a reverse 4-throw (RFT) pupilloplasty technique for endothelial keratoplasty. Methods: In RFT, the 9-0 polypropylene suture needle and a 26-Gauge needle pierce the iris tissue from the posterior surface to emerge on the anterior surface along the proximal and distal portion of iris tissue to be apposed, respectively. The 9-0 needle is threaded into the barrel of a 26-Gauge needle and is withdrawn from the eye. The suture loop is withdrawn and the suture end is passed through the loop 4 times. Both suture ends are pulled, which leads to the sliding of the loop inside the eye, thereby apposing the iris tissue with the knot lying on the posterior surface. Results: The procedure was performed in 11 eyes of 11 patients who were scheduled to undergo an endothelial keratoplasty procedure. The anterior segment optical coherence tomography image of the RFT pupilloplasty did not denote any presence of the suture in the anterior chamber or along the anterior surface of the iris plane. No incidence of primary graft failure or graft rejection during the entire follow-up period was reported in any of the eyes. Conclusions: RFT serves as an effective technique for performing pupilloplasty in endothelial keratoplasty cases without the presence of a knot or suture tail in the anterior chamber.

Zusammenfassung Hintergrund Ziel der vorliegenden Studie ist es, die Häufigkeit von Immunreaktionen und den Endothelzellverlust nach perforierender Keratoplastik (PKP) vs. Descemet Membrane Endothelial Keratoplasty (DMEK) bei Patienten mit Fuchs-Endotheldystrophie (FED) gegenüberzustellen. Patienten und Methoden In der vorliegenden retrospektiven Studie wurden insgesamt 962 Operationen (OPs; 225 Excimer-Laser-PKP und 727 DMEK) von 700 Patienten, die zwischen dem 28.06.2007 und dem 27.08.2020 in der Universitäts-Augenklinik des Saarlandes durchgeführt wurden, statistisch ausgewertet. Zum einen wurde die Prävalenz und der zeitliche Verlauf der aufgetretenen Immunreaktionen mittels Kaplan-Meier-Verfahren sowie die Auswirkungen der Immunreaktionen auf die Endothelzellen und die Hornhautdicke analysiert. Zum anderen wurden Endothelzelldichte, Pleomorphismus und Polymegalismus der Endothelzellen für die Zeitpunkte U1 = präoperativ, U2 = 6 Wochen postoperativ, U3 = 6 – 9 Monate postoperativ, U4 = 1 – 2 Jahre postoperativ und U5 = 5 Jahre postoperativ ausgewertet. Weiter erfolgte dazu jeweils eine statistische Testung auf Unterschiede zwischen den beiden OP-Arten sowie im longitudinalen Verlauf. Ergebnisse Insgesamt traten im beobachteten Zeitraum 54 Immunreaktionen auf, wobei die Wahrscheinlichkeit für eine solche bei der PKP mit 8,9% signifikant größer war als in der DMEK-Gruppe mit 4,5% (p = 0,011). Der Vergleich der beiden Kaplan-Meier-Kurven ergab im Log-Rank-Test ebenfalls einen signifikanten Unterschied zwischen den beiden OP-Techniken (p = 0,012). Der Endothelzellverlust durch die Immunreaktion war nur bei der PKP signifikant (p = 0,003). Auf alle OPs bezogen sank die Endothelzelldichte bei beiden OP-Techniken signifikant mit der Zeit (jeweils p < 0,0001), bei der DMEK stärker als bei der PKP (p < 0,0001). Außerdem war die Zelldichte bei der PKP insgesamt signifikant höher als bei der DMEK (p < 0,0001). Der Polymegalismus sank nur bei der DMEK-Gruppe signifikant (p < 0,0001). Der Pleomorphismus war im Durchschnitt bei der DMEK signifikant höher als bei der PKP (p < 0,0001). Schlussfolgerung Die Prognose der DMEK bei Patienten mit FED scheint nach Immunreaktionen günstiger zu sein als die der PKP, da nicht nur seltener Immunreaktionen auftraten, sondern diese auch milder verliefen. Die Endothelzelldichte war allerdings in der PKP-Gruppe während des gesamten Follow-ups signifikant höher.
