RESUMOObjetivo: Avaliar a eficácia de um novo marcador cirúrgico para ajudar na confecção da capsulorrexe anterior analisando o seu dimensionamento e formato, comparando com a capsulorrexe confeccionada manualmente de maneira livre. Métodos: Como experimento, 3 residentes (R3) de Oftalmologia do Hospital Universitário Onofre Lopes (HUOL) e 1 oftalmologista em treinamento, voluntários, realizaram (cada um) 10 capsulorrexes em olhos de porco enucleados. Em 5 olhos foi utilizado o marcador e em outros 5, não. Todos os olhos foram fotografados tendo ao lado uma régua para orientar e calibrar um aplicativo para a avaliação morfométrica do procedimento. O diâmetro alvo foi de 5 mm, cujo perímetro correspondente é 15,7 mm e a área 19,652 mm 2 . Foram avaliados em cada procedimento: os diâmetros máximo, mínimo e médio, o perímetro, a área e o desvio em relação ao diâmetro e quanto ao aspecto ideal. Resultados: No grupo utilizando o marcador o diâmetro médio foi 5,44mm (±0,89) contra 6,37mm (±0,67) (p=0,001), no grupo no qual não se utilizou o marcador; quanto ao perímetro, 17,52mm (±1,92) no grupo utilizando o marcador contra 20,14mm (±2,09) (p<0,001) sem o marcador e quanto a área, 24,73mm 2 (±1,92) com o marcador, contra 32,62mm 2 (±6,32) (p<0,001), sem o marcador. Em relação ao aspecto da capsulorrexe 1,26mm (±0,12), contra 1,21mm (±0,7) (p=0,09) e em relação ao desvio de curvatura: 0,87 (±0,05), contra 0,9 (±0,04), (p=0,06) respectivamente. Conclusão: O trabalho mostrou que o marcador avaliado é eficaz para auxiliar a confecção da capsulorrexe conduzindo a resultados melhores que o método a mão livre.Descritores: Capsulorrexe / instrumentação; Extração de catarata / instrumentação; Lasers

BACKGROUNDHuman resources like Doctors form the important part of all health systems. To achieve health objectives and goals, health resources should be distributed evenly among people irrespective of economic, social and geographical barriers. The aim of this study is to evaluate the surgical challenges faced by trainee Ophthalmologists and to compare the efficacy of two different methods of training; step-by-step method and one-step method are the two ways of training practiced to train ophthalmologists. This study compares the two ways of training by evaluating complications at the end of surgeries performed by trainee surgeons. Setting-Department of Ophthalmology, Index Medical College and Research Centre. Study Design-A Prospective Comparative Study. MATERIALS AND METHODSTime Period-1 st January 2017 to 31 st March 2017, 3 months. Cataract Surgery performed-Manual Small Incision Cataract Surgery performed within the above-mentioned period (80 SICS). Study was divided into 2 Groups accordinglyGroup 1-Including residents that were trained with the 'step-by-step' method (44 SICS). Group 2-Including residents that were trained with the 'one-step' method (36 SICS).1. Residents were evaluated for the ease of performing surgical steps. 2. Complications were recorded in terms of premature entry, buttonholing, running away of capsulorhexis, rhexis tear, zonular dialysis/dehiscence, posterior capsular rent/rupture, iridodialysis, vitreous loss. Both of the above-mentioned groups were compared according to various difficulties encountered in each approach and their management was done by the supervising surgeon. Patient was followed for 1 month. Data were listed and counted with Excel (Version 2007, Microsoft). Mean and Standard deviation was calculated. Chi-square and Student t-tests were used to assess statistical significance between groups. RESULTS 44 surgeries were done in "step-by-step" group of which 18 patients were male and 26 were female; and 36 surgeries were done in "one-step" group of which 20 were male and 16 were female. Mean age group of patient in "step-by-step" group was 64.22 ± 10.10 years and 63.50 ± 8.46 years in "one-step" group. Overall complication rate when compared between 2 groups were more in "onestep" group, which was significant in contrast to when individual complication rates were compared between two groups. CONCLUSIONThus to conclude, learning surgery is always an uphill task for a trainee. A great degree of temperament, practice and skills are required before proficiency is achieved. In our study, 2 approaches were practiced in learning SICS-"step-by-step" and "one-step"; and "step-by-step" approach was found beneficial for the beginners. Purpose: To improve the state-of-the-art teaching system by creating surgical videos with synchronised vertical 2-split screens. Methods: An ultra-compact, wide-angle point-of-view camcorder (HX-A1, Panasonic) was mounted on the surgical microscope focusing mostly on the surgeons' hand movements. In combination with the regular surgical videos obtained from the CCD camera in the surgical microscope, synchronised vertical 2-split-screen surgical videos were generated with the video-editing software. Results: Using synchronised vertical 2-split-screen videos, residents of the ophthalmology department could watch and learn how assistant surgeons controlled the eyeball, while the main surgeons performed scleral buckling surgery. In vitrectomy, the synchronised vertical 2-split-screen videos showed the surgeons' hands holding the instruments and moving roughly and boldly, in contrast to the very delicate movements of the vitrectomy instruments inside the eye. Conclusions: Synchronised vertical 2-split-screen surgical videos are beneficial for the education of young surgical trainees when learning surgical skills including the surgeons' hand movements.
