Strangulation of the umbilical cord due to combined amniotic band and true knot

Abstract: A case of fetal death due to an amniotic band and an umbilical cord true knot is presented. To the best of our knowledge the simultaneous occurrence of true knot of cord and amniotic band has never been previously reported and may shed light on the possible time of true knot formation in this case.

“…This hypothesis is supported by the observation that the frequency of knots in aborted fetuses is similar to that observed in mature deliveries 1 , 4 . The simultaneous occurrence of a true cord knot and an amniotic band in a case described by Elchahal et al further supports the case for an early origin of this abnormality 5 …”

“…From a purely practical point of view the formation of even a simple overhand knot requires that the fetal cord length be greater than the fetal head circumference plus half of the fetal body length 5 . In our case, which required that the fetus pass through two twisted loops of cord (Figure 2), these conditions could not have been met later in pregnancy.…”

“…We reviewed the Pubmed literature from 1992 to 2018, and totally 11 fetuses whose found umbilical cord entangled by amniotic bands, 8 died in utero and 3 survived. [2,[5][6][7][8][9][10] The poor prognosis of the fetus may be due to compression or constriction of the umbilical cord, which is also the reason of fetal intrauterine death in this group. The fetal hand and umbilical cord were simultaneously adhered by the amniotic bands, which caused the compression of umbilical cord.…”

“…6 Table 1 outlines the range of possible defects. 7,8 The severity of malformation is related to the gestational age at which the bands occur. If they occur within the first 45 days of gestation then the most severe malformations, involving the nervous system, face and viscera, occur.…”
