Strategies and foundations for scientific discovery in longitudinal studies of bipolar disorder

Abstract: Bipolar disorder (BD) is a complex and dynamic condition with a typical onset in late adolescence or early adulthood followed by an episodic course with intervening periods of subthreshold symptoms or euthymia. It is complicated by the accumulation of comorbid medical and psychiatric disorders. The etiology of BD remains unknown and no reliable biological markers have yet been identified.

“…Also, because of the relatively small sample sizes (maximum n = 329), additional nodes (e.g., demographics, comorbid psychiatric symptoms, traumatic experiences in adulthood) could not be included. Constructing more inclusive networks in BD may represent a valuable avenue for further studies, but will require larger cohorts 53 . Finally, the present networks were based on cross‐sectional data only.…”

“…Recordings must also cover manic, depressive, and euthymic phases, as well as longer-term illness progression. Designing and funding such studies is a pressing challenge [ 60 63 ].…”

“…Automated collection of selected measures by remote sensing using digital technologies offers promise, and such large scale data are amenable to artificial intelligence methodologies. Large scale and long duration international longitudinal cohort studies with repeated multiple biomarkers that span peripheral blood measures, electrophysiology, neuroimaging and cognitive neuro science, mirrored by deep clinical phenotyping, are likely to be a path forward -the planned BD 2 Integrated Network for Bipolar Disorder is an exemplar 9 .…”

“…Symp tom networks, extremes on symptom dimensions, and intense brain circuitry activations can be normal or abnormal depend ing on context. These proposals, whatever their merits, rearrange the symptomatic deck chairs on our nosological Titanic without addressing the root problem: i.e., that DSM psychiatric nosology is sinking due to lack of attention to the evolved nature of human normality, yielding invalid normal/disorder demarcations 9 .…”
