Stroke and peripheral embolisms in a pediatric patient with giant atrial myxoma

Abstract: Rationale:Cerebral stroke with peripheral embolism due to left atrial myxoma is very rare in children. Misdiagnosis may occur because of nonspecific symptoms in the heart.Patient concerns:We present a case of a 16-year-old boy who presented with ischemic stroke and embolisms in the lower extremity, caused by a giant left atrial myxoma.Diagnoses:Left atrial myxoma.Interventions:A giant gelatinous mass was completely excised, and the histopathological findings confirmed the diagnosis of atrial myxoma.Outcomes:Th… Show more

“…Embolic events occur in approximately 40-50% of patients with CM often to the brain due to fragments of the myxoma itself or surface emboli. In addition, embolization to the kidneys, spleen, aortic bifurcation, and the lower extremities is not infrequent [7]. Similarly, our patient had bilateral renal and splenic infarct due to tumor embolization along with the PICA territory infarct of left cerebellum and pons.…”

“…It often occurs in the brain owing to fragments of the myxoma itself or surface embolism. In addition, the embolization phenomenon may occur in the spleen, kidneys, aortic bifurcation, and lower extremities [ 12 ]. Transient ischemic attack (TIA) is the most common neurological presentation occurring in multiple sites with psychiatric symptoms.…”

“…Most cardiac myxomas occur in the left atrium and arise in the area of the fossa ovalis. 8 Complications of myxomas include thrombosis and emboli, atrial or ventricular arrhythmias, and atrioventricular valve obstruction. 7 The symptoms observed may be related to tumor location and size.…”
