Strong association between IgA nephropathy and HLA-DR4 antigen

Abstract: Although the etiology and pathogenesis of IgA nephropathy is not fully understood, it has been classified as an immune-complex induced glomerulonephritis caused by an immunological mechanism. Forty-two patients with primary IgA nephropathy underwent tissue typing for HLA-A, B, and DR antigens by the standard method using a microlymphocytotoxicity test. There was no significant correlation between HLA-A and B antigens and this disease. However, the frequency of HLA-DR4 was 57.1% in the patients with normal rena… Show more

“…We demonstrate that the frequencies of DRB1*04 was increased in patients with IgAN, consistent with other previous reports that HLA-DR4 was associated with IgAN in a Japanese population (P Ͻ 0.04), although the reported P value was not significantly small (Hiki et al 1982;Kashiwabara et al 1982). Moreover, the apparent association between HLA-DRA alleles and IgAN has not been clarified in the Japanese or any other ethnic group; we have demonstrated here for the first time a significant association of three SNPs in the HLA-DRA gene with IgAN.…”

“…In the past 2 decades, the association of HLA-DRB1 alleles and pIgAN has been studied in various ethnic groups [10,11,12,13,14]. In Japanese and French [19] cohorts, there was a strong association between pIgAN and the HLA-DR4 gene; a study of Japanese patients found that some haplotypes of the HLA-DRA locus had an important role in the development of pIgAN [30, 31].…”

“…In the past decades, there were many clinical studies which provided further evidence indicating a marked association of HLA gene polymorphism and pIgAN [2,3,4,5]. Earlier studies have shown that HLA-DR * 4 might be associated with pIgAN, especially in Japanese [10,11,12,13] and Chinese patients [14]. There is a significant association between HLA-DQB1 * 0301 and IgAN patients with a poor clinical outcome [15].…”

“…Clearly the ubiquitous nature of organic solvents makes it improb able that they are capable of causing glomerulonephritis in the absence of predisposing factors. Certain reports suggest a hereditary component in the development of glomerulonephritis with solvent exposure [17,27], while other forms of glomerulonephritis, namely Goodpas ture's disease [28], membranous nephropathy [29], and mesangial IgA disease [30] are well know to have HLA antigen associations.…”
