Structural Changes in the Tracheae of Preterm Lambs Induced by Ventilation

Abstract: ABSTRACT. Compliant immature airways sustain significant deformation following positive pressure ventilation. To evaluate the structural changes induced by in vivo positive pressure ventilation, tracheae of preterm lambs (107-116 d gestational age) were studied histologically.Nonventilated (group I: n = 7) and ventilated (group 11: n = 7) tracheal segments were excised and studied by histologic and morphometric techniques. Computerized image analysis was used to measure dimensions of tracheal wall components a… Show more

“…When the transmural pressure is not in the physiological range, it may undergo abnormal deformation, such as patients suffering from obstructive sleep apnoea (Isono et al, 1993) and those requiring mechanical ventilatory support (Hillman et al, 2007;Hirschl et al, 1996;Van Marter et al, 1992;Wolfson et al, 1992Wolfson et al, , 1998. This abnormal deformation and alteration of airway mechanical properties could be a potential cause of tissue damage leading to dysplasia and other associated chronic complications (Deoras et al, 1989;Scherrer et al, 2004;Spitzer et al, 1992). Moreover, abnormal deformation e.g.…”

“…The barotrauma and volutrauma resulting from mechanical ventilation have been reported to increase the dimensions of the trachea, including an increase in the anterior-posterior diameter and an overall increase in tracheal volume [1][2][3] . Clinically, this can result in elevated dead space ventilation and a higher work of breathing.…”

“…35 En la etapa neonatal, la distensibilidad total del sistema respiratorio varía entre 3 y 6 ml/cm H 2 O, dada fundamentalmente por el pulmón. 36,37 La pared torácica es muy complaciente, a diferencia de la etapa de adulto, cuando la distensibilidad de ambas es bastante semejante.…”
