Originally approved by the U.S. Food and Drug Administration (FDA) for its antihistamine properties, clemastine can also promote white matter integrity and has shown promise in the treatment of demyelinating diseases such as multiple sclerosis. Here, we conducted an in-depth analysis of the feasibility, safety, and neuroprotective efficacy of clemastine administration in near-term lambs (n = 25, 141–143 days) following a global ischemic insult induced via an umbilical cord occlusion (UCO) model. Lambs were randomly assigned to receive clemastine or placebo postnatally, and outcomes were assessed over a six-day period. Clemastine administration was well tolerated. While treated lambs demonstrated improvements in inflammatory scores, their neurodevelopmental outcomes were unchanged.

Asthma poses a significant global public health concern, particularly in urban centers where environmental pollutants and variable weather patterns contribute to heightened prevalence and symptom exacerbation. The Deonar dumping ground, one of Mumbai's largest landfills, releases a complex mix of particulate matter and hazardous gases, posing a serious threat to local respiratory health. Despite the urgency for comprehensive research integrating patient-specific data with localized weather and air quality metrics, such studies remain limited. This study addresses the critical research gap by investigating asthma risk factors near the Deonar dumping ground. Integrating detailed patient records with precise local weather and air quality measurements, our research aims to unravel the intricate relationship between environmental exposure and respiratory health outcomes. The findings provide crucial insights into the specific risk factors influencing asthma incidence and severity in this region, informing the development of targeted interventions and mitigation strategies. Employing a novel ensemble Deep Info Max -Self-Organizing Map (DIM-SOM) technique, our study compares its performance with various clustering algorithms, including SOM, K-Means, Bisecting K-Means, DBSCAN, and others. The novel ensemble DIM-SOM demonstrated superior performance, achieving a significantly higher Silhouette Score of 0.9234, a lower Davies-Bouldin Score of 0.1276, and a more favorable Calinski-Harabasz Score of 389723.6225 compared to other algorithms. These findings underscore the efficacy of the novel ensemble DIM-SOM approach in generating dense, wellseparated, and meaningful clusters, emphasizing its potential to enhance clustering performance compared to traditional algorithms. The study further emphasizes the need for proactive mitigation measures and tailored healthcare interventions based on the identified environmental risk factors.

Idiopathic nephrotic syndrome (INS) is one of the chronic kidney diseases that occurs in childhood. Starting from a few case reports in the 1950s–1970s and up to the present, the relationship between idiopathic nephrotic syndrome (INS) and the occurrence of atopic disorders in these patients has been discussed in many medical publications. These publications show that in certain patients, mainly children and adolescents, but also in adults with INS, various clinical symptoms and laboratory indicators of an atopic allergic process may be present. This process has been shown to involve Th2 lymphocytes, to have an excessive production of interleukins (IL-4, IL-5, IL-13), and to have an increased serum level of immunoglobulin E (IgE). This leads to the development of a systemic allergic inflammatory process, of which the kidneys can also become the effector organ. The coexistence of an allergic process which may adversely affect the course of nephrotic syndrome may be confirmed by the increased serum IgE level and the hypersensitivity reaction of the patient’s body to various environmental allergens (through the presence of allergen-specific IgE /asIgE/ antibodies to food, pollen, mould, dust, or other allergens in the blood serum). High concentrations of IL-13 and other plasma mediators of this inflammation (e.g., histamine, bradykinin) structurally and functionally damage the renal filtration barrier, and in particular, the function of podocytes in the glomeruli. Podocyte dysfunction disturbs the physiological process of plasma filtration in the glomeruli, leading to excessive protein loss in the urine. These disorders initiate the development of idiopathic nephrotic syndrome in these patients. This publication presents the coexistence of an allergic process caused by allergy to cow’s milk proteins and hypersensitivity to other allergens in a child with idiopathic nephrotic syndrome. This publication also assesses whether treatment of the allergic process coexisting with INS with an elimination diet (milk-free, hypoallergenic) and anti-allergic drugs affected the course and treatment of INS in this child.
