Subarachnoid Hemorrhage Associated with Intratumoral Aneurysm Rupture within a Posterior Fossa Hemangioblastoma: The Importance of Continued Surveillance for Cerebral Vasospasm

Abstract: Cerebellar hemangioblastomas are rare tumors of the neuraxis. Only seven cases of hemangioblastoma associated with a cerebral aneurysm have been reported. We report a case of a patient who presented with acute onset headache as a result of subarachnoid hemorrhage (SAH) and hydrocephalus. Radiographic workup revealed a hemangioblastoma with an intratumoral aneurysm. Preoperative cerebral angiography was performed for both embolization as well as characterization of the aneurysm. The patient underwent a suboccip… Show more

“…Vasospasm caused by subarachnoid hemorrhage (SAH) when aneurysms easily rupture results in a smaller inner diameter and increased blood flow velocity. 4,11,28 As a result, the WSS, calculated by formulas according to Poiseuille's law, would also in- crease. Thus, it is reasonable that high WSS in the parent artery is obtained when an aneurysm ruptures.…”

“…Only 19 cases of an intratumoral aneurysm have been reported in the past. 1,[6][7][8][9][10][11] The pathology of associated tumors was varied, including meningioma, glioma, and pituitary adenoma in 8, 5, and 3 cases, respectively. This diversity of tumor pathology suggests that there are numerous potential mechanisms for the emergence of this rare entity, and these potential pathogeneses appear to be complexly contributing.…”
