Subcarinal Lymph Nodes Should be Dissected in All Lobectomies for Non-Small Cell Lung Cancer—Regardless of Primary Tumor Location

Abstract: Subcarinal lymph node metastases were common despite frequent use of preoperative invasive mediastinal staging in lower-lobe, middle-lobe, and upper-lobe NSCLC. Subcarinal lymph nodes should be dissected or sampled routinely during operations for NSCLC to avoid understaging-regardless of preoperative invasive mediastinal staging and tumor location.

“…However, it is of prime importance to recall in addition the potential underlying significance of subcarinal LN tumoral involvement. We observed in a surgical series [2] that pN2 involvement concomitantly existed in the upper and lower mediastinum in 40% to 81% of patients according to the side and location within the lobe of NSCLC; the lower mediastinum mainly consisted in subcarinal N2. Multistation N2 involvement is known to worsen the prognosis.…”

“…But a recent study showed that the proportion of subcarinal lymph nodes involvement was also high in the right (21.8%) and left (25.8%) upper lobe tumors (22). Similarly, Eckardt et al concluded that lymph nodes #7 should be removed in any location of lobectomy to avoid understaging (23).…”

“…For left lingular division, Haruki et al 11 and Riquet et al 30 both reported that the highest incidence of metastasis was upper zone, for which we did not combine RML with RUL as non-lower lobe to analyze in spite of the resembling anatomic location. Eckardt et al 31 reported that unexpected subcarinal metastases were found in 5.9%, 5.1% and 1.6% of patients with a tumor in the lower, middle and upper lobe, respectively. Rocha et al 12 also concluded that tumor located in a lower lobe was significantly associated with upstaging.…”
