Subjective complaints and self‐evaluation of memory test performance in Questionable dementia

Abstract: Subjects with QD and mild AD showed higher frequency of subjective memory complaints, reflecting that the subjects still retain some ability to appreciate own memory function. However, the trend for over-estimation of performance in AD subjects suggests that the precision of estimation may be suboptimal. Moreover, depressive symptoms may affect the presentation of memory complaints and this factor should be carefully considered in future prospective studies.

“…The original (English) version of CSDD has been widely used in Western countries and been translated by a number of researchers into Chinese for use as a study tool in several studies (Chan et al, 2008;Tung & Chen, 2007;Wong et al, 2006). In Taiwan, the CSDD Chinese version has been utilized to measure the effectiveness of music therapy on cognition, behavioral problems and depression in 90 demented older adults in long-term care facilities (Tung & Chen, 2007).…”

“…In Taiwan, the CSDD Chinese version has been utilized to measure the effectiveness of music therapy on cognition, behavioral problems and depression in 90 demented older adults in long-term care facilities (Tung & Chen, 2007). In Hong Kong, the original CSDD was administered in a study (Wong et al, 2006) that evaluated subjective complaints and self-evaluated the efficacy of a memory test in 92 Chinese with questionable dementia and mild Alzheimer's disease and a two-phase epidemiologic survey on the prevalence of dementia in elderly Chinese (Chan et al, 2008). Despite such applications of the CSDD, no published evidence on the psychometric properties of the Chinese version of this tool was shown in these studies.…”

“…Awareness has been measured with clinician ratings [16, 17]; or based on discrepancy between self-report versus clinicians' impression [10] or versus informant report assessed with interview [10, 18] or questionnaires [7, 12, 14, 1921]; or discrepancy between self-report and objective performance [21, 22], which, depending on the task, measures self-monitoring or metacognitive abilities [3]. Each assessment method has limitations: Clare et al [21] detail difficulties with patient/informant discrepancies and the assumption that caregiver informants' or clinicians' reports are a better reflection of reality.…”

“…As demonstrated in this and in multiple other studies (Schmidt et al, 2001; Wong et al, 2006; Mitchell, 2008), there are variable degrees of accuracy in subjective reports of memory status as compared to standardized neuropsychological objective measures. The subjective report of memory difficulties in the subjects, including the controls and the patients with MCI and AD, shows reasonable correlations to the physician’s clinical judgment and to performance on neuropsychological tests.…”
