Successful Integration of Insecticide-Treated Bed Net Distribution With Mass Drug Administration in Central Nigeria

Abstract: In Africa anopheline mosquitoes transmit malaria and lymphatic filariasis (LF); insecticide-treated bed nets significantly reduce transmission of both. Insecticide-treated bed net provision to children under 5 (U5) and pregnant women (PW) is a major goal of malaria control initiatives, but use in Africa remains low because of cost and logistics. We therefore integrated insecticide-treated bed net distribution with the 2004 LF/onchocerciasis mass drug administration (MDA) program in Central Nigeria. Community v… Show more

“…Most continuous strategies partially sub- Of the six studies reporting ownership by > 60% of households, four used an uncontrolled cross-sectional survey design, surveying 300-3000 households 1-3 years after delivery began. 25,44,50,51 The other two used a before-and-after design in which approximately 2500 households were surveyed before and one year after ITN delivery during campaigns integrated with measles vaccination. 42,43,47,48 During the 1-2-year period between baseline and endline surveys, ITN ownership among households increased from 24.5% to 79% in one study and from < 1% to 55-70% in the other.…”

“…Of the 11 policy options recommended by the LF policy brief group, only 5 were accepted after the evidence synthesis. [51][52][53][54][55][56] These included the following: (i) Establishment of a standard protocol for diagnosis for LF; (ii) Vector control; (iii) Community directed distribution of long lasting insecticide treated bed-net (LLN) and Mectizan (Mass chemotherapy)/health education; (iv) Integrated control policy (onchocerciasis, schistosomiasis, and LF); and (v) Establishment of policy monitoring/evaluation (feedback mechanism)/with training and capacity building. The remaining options were rejected because of absence of sufficient evidence (Table 7).…”

“…[4][5][6][7][8][9] The NTDs are a diverse group of infectious diseases including STH infections, lymphatic filariasis (LF), schistosomiasis, onchocerciasis, and at least 13 others, prioritized by the World Health Organization as such, because they promote poverty and have a negative impact on pregnancy, child health and development, and adult worker productivity. Examples of synergism between malaria and NTD control programs include a reduction in LF transmission by the large-scale rollout of long-lasting insecticide-treated nets (LLINs) and indoor residual spraying (IRS) with insecticides for malaria control 10,11 and the distribution of LLINs and sulfadoxine-pyrimethamine for intermittent preventive therapy for pregnant women by Community-Directed Treatment (CDT) platforms that have been mobilized by the African Program for Onchocerciasis Control (APOC) 12,13 and the Global Program to Eliminate Lymphatic Filariasis (GPELF) 14 in Africa. Indeed, Nigeria has recently announced a nationwide integration of malaria and LF elimination efforts, which will use the CDT platform to enhance LLIN distribution and communication of education messages that promote LLIN use.…”
