Successful management of deep facial burns in a patient with extensive third‐degree burns: the role of a nanocrystalline silver dressing in facilitating resurfacing

Abstract: Full-thickness burns of the face are notoriously difficult to repair and their management poses a series of problems to the surgeon. We present the case of a 49-year-old man (only survivor of a catastrophic airport accident) with third-degree flame burns to >80% of total body surface area and extensive face damage who achieved a fully satisfactory outcome after a treatment plan based on gradual escharectomy followed by application of artificial dermis and, later, grafting with sheets of cultured keratinocytes.… Show more

“…Autologous grafting was performed at 23 ± 9 days after the last silver dressing application. Healing times recorded for burn wounds are consistent with the data previously reported by other authors [34,57,58]. Khundkar et al [20] demonstrated that Ag NP-based dressings are effective in reducing the time for re-epithelialization and the requirement for grafting in comparison to other treatment widely used in burn centers, such as silver sulfadiazine and 0.01% neomycin and polymyxin solution.…”

“…Whereas the clinical efficacy of Acticoat7™ has been the focus of interest in adults with superficial partial thickness burns [12] as well as in pediatric populations with different burn depths [13][14][15] and also in several previous case reports [16,17], our aim was to evaluate its value in the conservative treatment of superficial and especially deep partial thickness wounds of the face.…”

“…Patients were then treated with keratinocytes on Laserskin ® or with fibroblasts on Hyalograft 3D R ; the construct number for each patient was adapted to totally cover the wound, including the margins. Finally, the wounds were treated with a secondary dressing composed of nanocrystalline silver (Acticoat; Smith & Nephew Medical Ltd, UK; ).…”
