Successful Oocyte Retrieval After Follicular Fluid Aspiration in Suspicious of Ovarian Torsion

Abstract: During controlled ovarian stimulation, a 34-year-old woman complained of right lower abdominal pain after the decision to retrieve oocytes. Ovarian torsion was suspected and confirmed, so aspiration of follicular fluid was performed prior to oocyte retrieval for volume reduction of the affected ovary. Two days after that, oocytes were successfully collected. Four months later, the frozen embryo was transferred and got pregnant. In conclusion, it is possible to perform volume reduction before ovum pick up (OPU)… Show more

“…There has been one reported case of suspected ovarian torsion before trigger, but torsion was not confirmed on ultrasound or managed surgically [ 13 ]. In that case, a 34-year-old patient presented with symptoms suggestive of torsion and they proceeded with aspiration of follicular fluid for volume reduction of the right ovary, followed by a GnRHa trigger and oocyte retrieval 35 h later (see Table 1 for fertilization rates).…”

“…In a different management approach, Inoue et al describe performing 'follicular aspiration for volume reduction' for a 34-year-old patient with unexplained fertility who presented with presumptive torsion prior to oocyte retrieval. 17 They aspirated a 58 mm x 24 mm simple right ovarian cyst when the patient presented with right adnexal pain. The cyst fluid was evaluated microscopically, but no oocytes or cumulus cells were found.…”
