Successful Pregnancy Outcome in 5 Patients with Thrombotic Thrombocytopenic Purpura.

Abstract: Pregnancy is an initiating event for acute thrombotic thrombocytopenic purpura (TTP). Patients with a history of TTP have a high risk of relapse during pregnancy with associated maternal and fetal morbidity/mortality. We report 5 TTP patients who underwent serial clinical and laboratory monitoring (haemoglobin, platelets, blood film, reticulocytes, lactate dehydrogenase (LDH), ADAMTS-13 activity and IgG antibodies) and treatment within a specialist obstetric/haematology clinic, resulting in successful pregnanc… Show more

“…Our decision to use weekly infusions was empiric. Monitoring ADAMTS13 activity may guide the intensity of therapy, 9 but such data are not always available and plasma‐responsive TTP may occur in the absence of an overt deficiency of ADAMTS13 10 . Finally, while there is scant evidence to encourage the use of corticosteroids in this setting, data from Ezra and coworkers 3 support the use of antiplatelet agents for preventing relapse of TTP in pregnancy.…”
