Successful Treatment by Plasma Exchange in Guillain-Barré Syndrome with Immune Complexes

Abstract: A 18-year-old woman in the 7th month of pregnancy had a mild enteritis followed within 8 days by a typical Guillain-Barré syndrome with respiratory failure requiring ventilatory support and bulbar palsy. After induction of delivery and discouraging results with steroid therapy, she was treated by plasma exchange. After 2 sessions on alternate days, a recovery of motility was observed and respiratory failure was abolished. It is pointed out that the most significant improvement was recorded immediately after th… Show more

“…In this case, the removal of IC probably reflects the removal of the noxious agent. and relapsing variety [1,2,[5][6][7]; no report included more than 5 cases in either category of disease. Despite enthusiastic case reports, the inherent variability of the disease as well as occasional dramatic, spontaneous and rapid recoveries led some authors [10,11] to demand controlled studies before subjecting patients to this therapeutic modality which is not a benign procedure in acutely ill pa tients with respiratory depression and au tonomic instability [11].…”

“…Plasmapheresis was performed by discontinuous flow centrifugation, as previously described [9]; only 5 sessions were carried out by a manual technique. The volume of plasma removed was replaced by Haemaccel (Hoechst) and fresh frozen plasma.…”

“…PE was performed as previously described [13]: in 3 of 10 procedures, a two-vein technique could be used, but we were then forced to modify the exchange technique since only a single vein was available. At all sessions, 700 ml of the patient's plasma were first exchanged with 600 ml Plasmagel (Hoechst), whereas human group A fresh frozen plasma was used to complete the exchange.…”
