Successful Treatment of Calcified Patent Ductus in an Elderly Patient

Abstract

Successful surgical treatment of calcified patient ductus arteriosus (PDA) in a 57-year-old man is presented. A surgical approach was made through a median sternotomy, and a Fogarty balloon catheter was inserted through the PDA from the main pulmonary artery. After the balloon had been inflated and pulled back, the thrill due to the PDA disappeared. The catheter was secured in place using a purse-string suture and a tourniquet. Cardio pulmonary bypass was then started. The PDA was detached from the pulmonary artery with a surrounding cuff of the pulmonary arterial wall. The cuff of the pulmonary arterial wall was then sutured and closed, with removal of the balloon catheter just prior to the final closure. The pulmonary artery was closed using a pericardial patch. This procedure allows complete division of the PDA without any manipulation of the calcified PDA and weakened aorta.
