Successful treatment of cardiac haemangiomas with oral propranolol: a case series of two patients

Abstract: BackgroundCardiac haemangiomas are extremely rare tumours with equivocal surgical outcomes. Haemangiomas appearing on other sites of the body have been successfully treated with oral propranolol. To the best of our knowledge, such treatment has not been tried to date for cardiac location of haemangiomas.Case summaryWe report two cases of neonatal cardiac haemangiomas, and we describe their presentation and characteristics, as well as how these were successfully treated with oral propranolol, with complete regr… Show more

“…Biopsy of the mass was crucial to establish the final diagnosis, displaying typical morphological and immunohistochemical features and the absence of nuclear atypia or proliferation. The benign diagnosis and the impossibility of surgically resecting the masses prompted conservative therapy with oral propranolol, based on the evidence that this drug has induced regression of neonatal cardiac hemangiomas ( 10 ).…”

“…Tumor histology is the most significant predictor for mortality [ 9 ]. Alternative approaches, such as medical treatment with propranolol, due to having been established for a long time in the treatment of skin hemangioma, have been reported [ 10 ]. However, their efficacy and long-term outcomes are uncertain.…”

“…In cases, where the tumor is unresectable or has low chance of malignancy, long-term stability without obstruction or compression of adjacent structures has been observed [ 11 ]. Hemangiomas can be treated with beta-blockers, which have shown a decrease in tumor volume [ 10 , 12 ]. Heart transplantation is considered as a last resort for the treatment of primary non-operable benign cardiac tumors [ 13 ].…”

“…Clinically, propranolol is the drug of choice for the treatment of liver or skin haemangiomas, and there have been successful cases of oral propranolol in the treatment of infant cardiac haemangiomas. 9 , 10 However, there is no alternative treatment for adult cardiac haemangioma except surgery. Many scholars believe that cardiac haemangioma has the possibility of embolism, rupture and sudden death, therefore, in symptomatic children and adults, surgical resection is recommended.…”
