Successful Use of Vacuum-Assisted Closure Therapy for Leg Ulcers Caused by Occluding Vasculopathy and Inflammatory Vascular Diseases – A Case Series

Abstract: Background: Leg ulcers caused by vasculitis, small vessel occlusion or other rare conditions often prove to be very difficult to treat. Despite polypragmatic, systemic and localized therapy, many of these wounds are progressive and characterized by severe pain. Methods and Results: We here portray the cases of 5 patients with ulcers resistant to systemic therapy for the underlying disease, who were treated successfully using vacuum-assisted closure (VAC) for wound management. We present the advantages and disa… Show more

“…The extensive studies on the established use of the VAC technique in the wound-healing phase of granulation have resulted in guidelines that recommend VAC therapy in acute and chronic wounds [7,8,9,10,11,12], burns [13], diabetic ulcers [14, 15] as well as open foot wounds [16], vasculitis- or occluding vasculopathy-induced ulcers [17]. However, there have been very few reports on the postoperative application of VAC therapy [18, 19].…”

“…Thus, the utility of such tissue material is primarily limited to histological studies. Vasculopathy represents a major factor that complicates cutaneous wound healing (4,5). The objective of this study was to develop an approach to specifically laser capture blood vessels from standard 3-mm human wound biopsies such that the captured blood vessel tissue element would lend itself to genomic screening and verification of candidate genes, using quantitative PCR.…”

“…It has also been used to heal wounds by secondary intention or as a bolster type dressing to improve skin graft take, with Ghersi et al (21) demonstrating healing of PG wound by secondary intention specifically. The relevance of this modality to chronic wound management must not be understated because it not only provides an effective barrier to secondary infection, but also requires less frequent dressing changes and improves the local microcirculation, resulting in healthy granulation tissue formation (22).…”
