Sudden Death Related to Diabetes Mellitus: Current and Emerging Relevance to the Forensic Pathologist

Abstract: While diabetes mellitus (DM) has historically accounted for substantial worldwide morbidity and mortality, new and evolving epidemiologic trends are forcing the scientific community to view it as a type of emerging disease. This review will summarize the pathophysiology of the disease, present an update of current national statistics and changing epidemiologic patterns, discuss how DM might specifically lead to acute deaths falling under the jurisdiction of the medical examiner/coroner, and suggest a medicoleg… Show more

“…Analyzing the impact on mortality from diabetes for women versus men or separated by age does not produce statistically significant result. An important caveat for the diabetes analysis is the fact that diabetes does not lead to a sudden unexpected death, but rather leads to a development of many symptoms which can result in individual's death (Ely, 2016). Deaths caused by underlying diabetes can manifest in deaths due to heart attack, heart failure, stroke, kidney failure and coma, resulting in death being recorded as a death due to cardiovascular disease and not diabetes.…”

“…A previous clinical study showed that type 1 DM patients less than 40 years of age have a 4to 10-fold rate of sudden and unexpected death compared with the general population; thus, this condition should be always considered as one of the possible causes of death in this population. [36][37][38] Both postmortem pathologic and serologic investigations targeting DM should be emphasized, even in younger victims.…”

“…Hypoglycemia is thought to be the cause of death for 2–4 percent of people with diabetes mellitus. It could explain the "dead-in-bed syndrome," which refers to the mysterious deaths of type 1 diabetics that happen at night [ 4 , 7 ]. The signs of low blood sugar levels can change over time and vary from person to person [ 8 ].…”
