SUN-233 The state of CKD care in Canadian primary care: a retrospective analysis of a national database

Abstract: evaluation of the usability, practicality and convenience of this new innovative approach to CKD care. Evaluations would include the impact on patient experience and satisfaction, the assessment of such outcomes as the reduction in the number of (unnecessary) referrals to Nephrology Clinics, any healthcare cost savings and overall efficacy of this new system of IT-based CKD care paradigm.
