Osteoarthritis (OA) is a chronic condition characterized by degeneration of the articular cartilage and subsequent bone hyperplasia. With the increasing prevalence driven by factors such as aging and obesity, OA poses a growing health and economic burden. Current treatments, including NSAIDs and corticosteroid injections, offer limited efficacy and often result in adverse effects, highlighting the urgent need for novel therapeutic approaches. Emerging research has identified hydrogel microspheres as a promising solution for OA management. These microspheres, with their tunable physicochemical properties and biocompatibility, provide a conducive environment for cartilage regeneration and sustained drug delivery. Unlike bulk hydrogels, microspheres offer enhanced injectability, uniform distribution, and reduced tissue damage. This review discusses the recent advancements in hydrogel microsphere technology, focusing on their fabrication, unique mechanisms, and therapeutic applications in OA. Additionally, we address the challenges and future directions for clinical translation, with the aim to optimize the efficacy of hydrogel microspheres in OA treatment.

Osteoarthritis (OA) is a common chronic inflammatory disease that leads to disability and death. Existing therapeutic agents often require frequent use, which can lead to drug resistance and long‐term side effects. Polyphenols have anti‐inflammatory and antioxidant potential. However, they are limited by their short half‐life and low bioavailability. This work presents a novel pure polyphenol capsule for sustained release of polyphenols, which is self‐assembled via hydrophobic and hydrogen bonds. The capsule enhances cellular uptake, scavenges reactive oxygen and nitrogen species, reduces inflammatory markers, and remodels the OA microenvironment by inhibiting the p38 MAPK pathway. The capsule overcomes the limitations of short half‐life and low bioavailability of polyphenols and achieves single‐dose cure in mouse and dog OA models, providing an optimal therapeutic window for OA repair. Taking advantage of simple manufacturing, convenient administration, and pure polyphenol composition, these capsules show great potential for clinical treatment of osteoarthritis and chronic inflammatory diseases.

Osteoarthritis (OA) is a chronic degenerative joint disease characterized by the degradation of articular cartilage. Recent studies have demonstrated that chondrocyte ferroptosis plays a crucial role in the progression of OA. Consequently, developing nanomedicines that suppress chondrocyte ferroptosis is a promising strategy for OA treatment. However, there are few reports on nanomedicines specifically targeting chondrocyte ferroptosis for OA therapy. In this study, Curcumin‐loaded nanoparticles (Cur‐NPs) are fabricated to suppress chondrocyte ferroptosis by regulating reactive oxygen species (ROS), ferrous ion (Fe2⁺), and Acyl‐CoA Synthetase Long‐Chain Family Member 4 (ACSL4) levels of chondrocyte. This is achieved by combining the functions of curcumin and an amphiphilic block copolymer with ROS scavenging and iron‐chelating properties. The in vitro anti‐ferroptotic effects of Cur‐NPs are thoroughly investigated. The findings indicate that Cur‐NPs decrease the expression of ferroptosis markers such as ROS, Fe2⁺, and ACSL4, while protecting the mitochondrial membrane potential of chondrocytes. Additionally, Cur‐NPs attenuated lipid peroxidation in chondrocytes. Furthermore, Cur‐NPs significantly reduced the expression of the catabolic factor Matrix Metallopeptidase 13 (MMP13) and increased the expression of the anabolic factor Collagen type II (Col II) in vitro. This study demonstrates that Cur‐NPs exhibit enhanced chondroprotective effects through anti‐ferroptotic actions, presenting a promising approach for inhibiting chondrocyte ferroptosis using bioactive nanomaterials in OA treatment.
