Supraorbital keyhole approach for anterior circulation aneurysms

Abstract: AIm: Supraorbital keyhole approach provides access to the major part of the anterior circulation aneurysms. Herein, our surgical experience of supraorbital keyhole approach and its some modification have been proposed. mAterIAl and methOds: Out of a series of 76 patients harboring 80 aneurysms operated on via a supraorbital keyhole approach with a superciliar or front wrinkle skin incision, there are 70 patients with subarachnoidal bleeding, others are nonruptured aneurysms, Intraoperative rupture occurred in … Show more

“…There was no statistically significant difference in the mRS between the two groups ( P = 0.137), similar to a previous study by Park and Park (2). The reason there was no difference between the two approaches is because although the skin incision, craniotomy size, and dura openings are different, both of the approaches use the standard microsurgical procedure to clip the aneurysm (14), as mentioned earlier.…”

“…There was only one patient in each group that developed frontal numbness. During the skin incision using the supraorbital approach, the incision remains superficial to avoid injury to the supraorbital nerve (14).…”

“…[6,8] PcA clipping mostly requires anterior clinoidectomy, optic canal opening, and resection of the dural ring, therefore standard or modified pterional approach including the Dolenc skull base technique is preferable; however, a subset of PcA can be clipped through minimal invasive keyhole mini-craniotomy. [11] e supraorbital keyhole (SOK) surgery provides adequate aneurysmal access while minimizing trauma to the surrounding structures, including the skin, bone, dura, and, most importantly, the brain, [5] yet not applicable to all PcA.…”
