Supratentorial metastasis of medulloblastoma in adults

Abstract: Two adults, 31 and 20 years of age, developed supratentorial metastasis 3½ years and 11 months, respectively, after gross total removal of their posterior fossa medulloblastoma. The first case developed spinal metastasis as well. Both had undergone craniospinal irradiation. Case 1 underwent laminectomy and case 2 underwent craniotomy because their presenting symptoms required so.

“…The similarities in morphology, gene expression, and a conserved SHH signature in metastatic tumor in murine and PDX models, all reminiscent of SHH MB metastasis in patients, indicate that ATOH1-driven tumorigenesis provides an opportunity to study metastasis of naïve SHH MB in humans (4, 7, 3842). However, the role of ATOH1 in recurrence (local or distant relapse) remains unclear.…”

“…14 Several studies have documented the spread of MB locally to different brain regions as the ventricular system, the occipital pole, and subarachnoid space. 15 16 17 In this case, we observed intraventricular metastatic nodules and CSF seeding affecting multiple spine levels and the cauda equina. Moreover, after treatment for 1 year, the patient developed several lung lesions that are mostly metastatic.…”

“…Kumar et al reported two cases, aged 31 and 20 years, with supratentorial metastases occurring 3½ years and 11 months after the complete removal of posterior fossa medulloblastoma. Both patients underwent craniospinal irradiation; notably, the first case also developed spinal metastasis [ 13 ].…”
