Surface anatomy for implantation of external ventricular drainage: Some surgical remarks

Abstract: Background:External ventricular drainage (EVD) is an emergency process intended to reduce intracranial hypertension resulting from the obstruction of cerebrospinal fluid (CSF) flow. This creates a temporary situation to extract CSF that cannot pass through normally. Knowing the surface anatomy for EVD implantation is important to prevent its inadvertent complications. The external landmarks have been designed in this anatomic study to review the classical landmarks and come up with new landmarks to improve thi… Show more

“…This means that the best practice for the positioning of the drainage system corresponds to the level of the external auditory canal of the ear, corresponding to the line between the eyebrows seen from the side angle. It is adjusted to the height specified by neurosurgeons according to the clinical conditions of each patient so that CSF is more reliable (19,34) .…”

“…A high detail setting with a layer resolution of 0.15 mm and a 20% infill density was used to maintain the minimum hardness required for the simulation. Then with the printed model, the burr hole was made with a craniotome at Kocher's point, located by completing the following measurements: 10 cm from the nasion following the midline, then 3 cm lateral to the midline and confirming that it is 1–2 cm anterior to the coronal suture, this being the ideal point for catheter insertion, 30 corresponding to the intersection of the lines that join the projection of the external auditory canal with the ipsilateral internal canthus.…”

“…Then, with the patient's model placed in the simulator in a dorsal decubitus position and with the head slightly flexed with 0 degrees of rotation, 30 the spatial location of the Kocher point was recorded. This was done with the help of the optical tracker and by palpating the edges of the burr hole with the stylet and identifying their central point.…”
