the changes in lung function following surfactant treatment can only be explained by initial stabilisation of already aerated alveoli followed by recruitment of new gas exchange units as mechanisms involved in mediating the effect of surfactant on gas exchange. However, since no significant correlation between changes in functional residual capacity and improvement in arterial-to-alveolar oxygen tension ratio was seen, other effects of surfactant must be considered. These include local and/or systemic changes in haemodynamics.

We studied the possibility of improving lung volume and therefore clinical outcome in premature newborn lambs by increasing the inspiratory volumes during the first minute after birth. Sixteen lambs from eight were delivered by hysterotomy after 130-131 days' gestation. In eight lambs the lungs were inflated with a bag with a sustained inspiratory inflation (SI) of 5 s and expiratory time of 5 s during the first four inflations after cord clamping and then mechanically ventilated. Their siblings did not receive SI and served as a control group. At 8 h postnatally, the SI and control groups showed the following results (mean +/- SEM): mean airway pressure 14.8 +/- 1.8 cmH2O versus 11.9 +/- 1.1 cmH2O, PaO2 41.5 +/- 7.3 kPa versus 31.3 +/- 7.7 kPa, alveolar-arterial oxygen gradient 359 +/- 55 mmHg versus 437 +/- 58 mmHg. Clinical course, incidence of pneumothorax, oxygenation index, total static compliance, parenchymal-alveolar air area ratio or mortality rate were not different. There was no significant difference between the two groups at this time or at any other time during the experiments.

We tested the hypothesis that the initial functional residual capacity (FRC) of preterm infants with hyaline membrane disease (HMD) could predict the response to surfactant replacement (Survanta, 4 mL/kg/dose), with a better initial FRC being correlated with a greater improvement in PaO2, a/A PO2 ratio, and FRC. Thirty-four preterm infants were studied on 41 occasions. FRC and arterial blood gases were measured immediately prior to treatment. FRC was measured by the helium dilution method. Arterial blood gases were measured again after 30, 60, and 120 minutes. FRC was measured after 120 minutes. Twenty-seven treatments resulted in an increase in PaO2 >10 mmHg (responders); 14 did not (nonresponders). There was no correlation between initial FRC, change in FRC, and change in PaO2 (r2 = 0.07). These results suggest that there is no relationship between initial FRC and response to surfactant treatment.
