Surgical management of large bowel obstruction due to colonic cancer

Abstract: One-stage primary resection and anastomosis of the large bowel, is a feasible option in cases of emergency. LBO caused by colonic carcinoma can be performed with acceptable morbidity and mortality whenever comorbidity of the patient is not a contraindication.

“…Obstruction occurs in the small bowel in 60% of cases and is most commonly due to postoperative adhesions (38.5%) and hernia (21.7%) [1,2]. Occlusion of the large bowel is less common and differs in aetiology (generally malignant), pathophysiology, treatment and prognosis, with a mortality rate around 20% [3].…”

“…Multidetector-row computed tomography (MDCT), as the numerous studies in the literature demonstrate, is the most widely used imaging modality in the evaluation of large-bowel obstruction. The technique has a sensitivity of 96%, specificity of 94% and positive predictive value (PPV) of 100% [2,13]. MDCT, when associated with multiplanar reconstructions (MPR) and volume rendering (VR), is accurate in defining the presence, level and he nature of the occlusion and has a sensitivity of 83%, specificity of 93% and accuracy of 91% in identifying the presence of ischaemic complications [14][15][16].…”

“…In 7 patients (30.44%), temporary colostomy had to be resorted & 8 out of the 23 cases (34.78%) were subjected to end colostomy or ileostomy as definitive procedures. In a study conducted by Gatsolius et al [11], 21 patients with Large Bowel Obstruction underwent emergency surgery. Pateints with Large Bowel Obstruction caused by obstructive malignant colonic lesions underwent either with one-stage primary resection and anastomosis (14 patients, 67%) or twostage operation (7 patients, 33%).…”
