Abstract: Initial findings with this surgical procedure indicate a potential treatment for late CBS; however, additional studies are needed before a firm conclusion can be drawn.

“…5,12,13 Patients with late postoperative CBS have decreased visual acuity, impaired contrast sensitivity, and glare disability because of PCO and accumulated fluid in the bag. [3][4][5][6]10,[12][13][14] In our cases, no patient reported these symptoms except gradually decreased vision. We think the milky-white fluid may have a more negative effect on contrast sensitivity than visual acuity, such as early nuclear cataract.…”

“…14 The cause of failure is reported to be that the laser beam could not be accurately focused on the posterior capsule because of the intense opacity of the contents in the distended capsular bag. 14 We did not encounter such difficulties, and the Nd:YAG laser posterior capsulotomy was successful in all cases because our patients had PCO and posterior capsule distension. After Nd:YAG capsulotomy, capsular block resolved in all eyes and the CDVA improved in 13 eyes (86.7%).…”

“…4 These figures are higher than PCO rates found with previous models of the IOL. 5 Poor PCO rates (64%) have also been noted in the ThinOptX Ultra Choice (ThinOpt, Inc.) microincision IOL, 6 suggesting that microincision IOLs have poor PCO performance.…”

“…The treatment strategies for CBS include a wait-andsee approach, antiinflammatory medication drops, 2 slitlamp needle revision of the capsular block, 3 surgical revision with aspiration of the cortical masses and the fluid, 4 and anterior or posterior capsulotomy using the Nd:YAG laser. 5 In a previous report, 6 we were reluctant to use posterior laser capsulotomy because of the risk for phacoanaphylactic endophthalmitis.…”

“…3,5 CBS may be noncellular, inflammatory, or fibrotic; 3 our case was a combination of inflammatory and fibrotic. We speculate that the anterior membrane occluded the pupil and peripheral iridectomy.…”
