Surgical Repair of Internal Carotid Artery Aneurysm: Case Report

Abstract: Extracranial carotid artery aneurysms are extremely rare aneurysms. Treatment options are conventional open surgery, endovascular surgery, and conservative treatment. Surgical treatment of such aneurysms is challenging and it is important to determine the treatment strategies wisely. There is still no definite association for treatment modalities. In this case report, we present open surgical treatment of a 58-year-old female patient with internal carotid artery aneurysm. The patient was admitted to our clinic… Show more

“…[5] Although there is no evidence of its long-term effectiveness, various studies have demonstrated that surgical procedure is the most effective treatment modality for cervical aneurysms. [2,8,15] However, it has some limitations, including cranial nerve damage, which occurs in 11.8% of cases after surgical procedures. [16] e other problem is that the distal ICA is more difficult to manage in lesions near the skull base.…”

“…[17] e most important problem in endovascular procedures is reprocessing the aneurysm and restenosis within the stent. [5,8] Endovascular procedures should be recommended for lesions with a higher position, shorter distal ICA, and no healthy vessels, such as pseudoaneurysms, traumatic aneurysms, or dissecting aneurysms. In our case, we selected the surgical procedure because the aneurysm was proximal with sufficient length of the distal ICA, and the patient had no history of trauma, infection, or radiation therapy.…”

“…The major challenge is proper exposure for vessel control at the skull base. In both the above studies and other isolated case reports, various techniques have been described to get adequate exposure [ 9 - 10 ]. Endovascular treatment has also been proposed as the safer alternative technique, with the sacrifice of the vessel following balloon occlusion testing or endovascular stent placement with distal vessel flow maintenance [ 11 ].…”
