Surgical repair of unilateral absence of pulmonary artery in children with pulmonary hypertension: a single-center retrospective study

Abstract: Background: The early diagnosis of unilateral absence of pulmonary artery (UAPA) in children offers an opportunity for effective intervention. Due to the lack of clinical evidence, a consensus regarding surgical treatment has yet to be reported. The aim of this study is to evaluate the effectiveness and safety of pulmonary artery (PA) reconstruction with a "two-segment" technique to repair UAPA in patients with pulmonary hypertension. Methods: Intraoperatively, the ligamentum arteriosum connecting the innominat… Show more

“…Sildenafil is recommended for the treatment of pulmonary hypertension ( 10 ). Also, patients with UPAA and pulmonary hypertension should undergo surgical reconstruction and pulmonary artery rehabilitation ( 12 ). Pulmonary angiography and embolization can be performed in patients with hemoptysis because of the chronically hyperperfused vessels on the contralateral side of the UPAA ( 1 , 4 , 13 ).…”

“…(17) An alternative is the interposition of a tube graft with autologous pericardium or prosthetic material and mobilization with anastomosis of the affected PA branch to the main PA. (17) Besides, there are other approaches such as embolization, lobectomy or pneumectomy in cases of recurrent haemoptysis or closure of selected collateral arteries. (1)(17) (22) In our centre, one patient underwent corrective surgery of a CHD and another right lobectomy. The remaining three patients that did not undergo any intervention for the UAPA had remained asymptomatic with no PHT.…”
