Surgical resection of a cardiac lipoma of the right ventricle

Abstract: Cardiac lipomas are rare, benign, encapsulated tumors which predominantly appear outside of the heart and less frequently inside the cardiac chambers. We describe a case of a right ventricular cardiac lipoma in a 72-year-old female patient who presented with palpitations. Surgical considerations, diagnostic modalities, and up-to-date literature are also presented and discussed.

“…Cardiac lipomas can be located in any part of the heart and typically originate from the epicardial fat or pericardial fat. They are surrounded by fibrous tissue and contain a small amount of myocardial tissue, as well as components of connective tissue ( 31 ). Lipomas located in the myocardium are usually small with a complete capsule, and occasionally grow on the mitral or tricuspid valve ( 31 ).…”

Lipomas located in the myocardium are usually small with a complete capsule, and occasionally grow on the mitral or tricuspid valve ( 31 ). The most common location is the atrial septum, followed by the endocardium of the right atrium and left ventricle ( 41 ), while in a few cases the lipomas are located in the myocardium, subepicardium and pericardium ( 29 ).…”

“…La intervención quirúrgica suele ser la opción terapéutica; sin embargo, debido a la baja incidencia de la enfermedad, no existen pautas claras sobre el momento más adecuado para realizarla (17) . Se puede efectuar un seguimiento periódico para pacientes asintomáticos o directamente la intervención quirúrgica para aquellos con arritmias, insuficiencia cardíaca, secuelas tromboembólicas, o en los que no es posible descartar la malignidad del tumor (17) .…”

“…Cardiac lipomas are rare primary cardiac tumors, the majority of which are asymptomatic ( 1 ). Clinical manifestations in affected patients generally vary as a function of tumor size and location, with some patients experiencing symptoms attributable to the compression of heart chambers by a large tumor volume or the involvement of the myocardium or heart valves that ultimately lead to their presentation for symptom.…”
