Surgical treatment of low and intermediate grade lung net

Abstract: In central typical carcinoid, the use of lung-sparing bronchoplastic techniques could influence local recurrence in some cases. This observation demands the intraoperative pathologic verification of an adequate surgical margin by frozen section. Peripheral typical carcinoids have been surgically treated, occasionally, by sublobar resection. However, in peripheral atypical carcinoid after a limited sublobar resection the observed increase of the probability of local recurrence makes it, in our opinion, not advi… Show more

“…Overall, distant metastases arising from resected typical carcinoids are rare (~3%). [40, 41] To our knowledge, this is the first documentation that when distant metastases do occur, typical carcinoids have the capacity to develop elevated – and in some instances substantially elevated – proliferation rates. These data indicate that proliferation rate is a dynamic variable during metastatic progression, and the diagnosis of typical carcinoid in the primary tumor does not assure low proliferation rate in metastases.…”

“…LCs occur during the fifth or sixth decade of life for TC and a decade later for AC. [4][5][6][7][8][9][10] ThC is an extremely rare tumour accounting for an ageadjusted rate of 0.02/100 000-0.18/100 000/year in the European or USA populations, respectively. 3,11 In the Netherlands cancer registry or the Surveillance, Epidemiology and End Results (SEER) programme database, <0.5% of all neuroendocrine neoplasms were ThC, representing 5% of thymic tumours.…”

“…noted the same. 14 The presence of nodal involvement carries significant implications. Firstly, lymphadenopathy on pre-operative imaging should prompt the clinician to consider the tumour as being atypical in nature, and therefore, more aggressive in its oncological behaviour.…”
