“…Endarterectomy was done in 20 per cent of the patients in addition to bypass grafting to achieve maximal revascularisation; as we have found that this does not adversely affect the clinical result, we now do this wherever necessary . Bypass grafting can be safely combined with aneurysmectomy in the treatment of post-infarction ventricular aneurysms (Donaldson et a!., 1976). All but 1 of the 18 patients in this series who underwent the combined procedure had excellent symptomatic relief.…”

“…There are a small number of cases with antemortem diagnosis in the literature (4), but their true incidence is probably unknown. Gated blood pool scans can be useful for detecting patients with left ventricular aneurysm (11,15); symptomatic patients with localized disorders of contraction should then proceed to cardiac catheterization if surgery is indicated (2). As the number of cardiac cycles recorded with this equilibrium technique can be controlled by the operator, this non-invasive method allows precise anatomical delineation and functional assessment of left ventricular function (3,9).…”

“…Such patients are candidates for corrective surgery provided: 1) coronary cineangiography reveals adequate circulation or one that can be treated by a saphenous‐vein transplant, and 2) the left ventricle is adequate in size and contractility. A low operative mortality and good clinical results have been reported in 15 patients of the 60–71 age group (16).…”
