Surgically treated reactive arthritis of the ankle after COVID-19 infection: A case report

Abstract: A 37-year-old man developed right ankle pain and swelling six days after being diagnosed with coronavirus disease (COVID-19). Despite conservative treatment, his ankle symptoms persisted. Magnetic resonance imaging and computed tomography showed synovial hypertrophy and bone erosion in the ankle. Following arthroscopic synovectomy, performed 69 days after the COVID-19 diagnosis, the pain improved significantly. The clinical course was consistent with that of reactive arthritis following severe acute respirator… Show more

“…The overwhelming majority of patients did not have a personal history of preexisting joint symptoms or autoimmune disease. Pertinent comorbidities included preexisting RA in one of the cases described by Alivernini et al 9 as well as one of the cases described by Derksen et al, 17 gout with recurrent acute arthritis in three of the cases described by Lopez-Gonzalez et al, 29 gout in each of the cases described by Ouedraogo et al 35 and Shimoyama et al, 40 unspecified recurrent acute arthritis in one of the cases described by Lopez-Gonzalez et al, 29 remote history of septic arthritis in one of the cases described by Sinaei et al, 42 chronic intermittent monoarticular joint pain in the case described by Kuschner et al, 27 sarcoidosis in one of the cases described by Derksen et al, 17 and autoimmune hypothyroidism in two cases described by Colatutto et al. 14 A comprehensive list of the comorbidities reported for each patient can be found in Table 1 .…”

“… 31 , 40 Neves et al 31 inserted a drainage catheter in a patient presenting with presumed septic arthritis with intramuscular collections with joint continuity noted on imaging. Shimoyama et al 40 performed arthroscopic synovectomy in a patient with reactive arthritis that was unresponsive to a trial of NSAIDs and intraarticular steroid injection. It is important to note that most reports lacked details regarding treatment duration.…”

“…COVID-19 chest computed tomography images such as ground glass opacities may resemble rheumatologic SAD including SLE and RA with extensive lung involvement, making the diagnosis of viral infection challenging [ 135 ]. Due to inflammation, bone erosion leading to rheumatoid nodules may be detected [ 136 ], and viral SARS-Cov2 RNA retrieved in the synovial fluid [ 137 , 138 ]. The shift from reactive arthritis following COVID-19 infection to RA remains exceptional, which supports the possibility that additional genetic and environmental factors are required (Arleevskaya et al, 2021).…”

“…This could be related to early stages of disease or because medications used to treat other manifestations of COVID-19, such as corticosteroids or hydroxychloroquine, may blunt the severity of the synovitis ( 10 ). The presence of erosions may suggest an underlying rheumatologic disease such as RA ( 34 ). Similarly, US can help detect synovial thickening, which may be heterogeneous or hypoechoic relative to other tissues.…”
