Surveillance of Barrett's Esophagus and Mortality from Esophageal Adenocarcinoma: A Population-Based Cohort Study

Abstract: Participation in a surveillance program for BE, but only if adequately performed, reduces mortality from EAC. Nevertheless, it remains to be determined whether such a program is cost-effective, as more than 90% of all EAC patients were not known to have BE before diagnosis.

“…As esophageal cancer survival is stage dependent, these studies suggest that survival may be enhanced by endoscopic surveillance. Recent work from a large Dutch population-based cohort study confi rmed that there is a survival advantage for EAC in patients who received adequate endoscopic surveillance compared with patients who were not participating in endoscopic surveillance ( 100 ). Similarly, a large Northern Ireland populationbased study found that in patients with EAC and a prior diagnosis of BE, survival was enhanced, tumor stage was lower, and tumor grade was lower compared with patients without a prior diagnosis ( 101 ).…”

“…Studies are ongoing to determine how to prevent GERD and Barrett esophagus from progressing to esophageal cancer. Treating GERD with proton pump inhibitor drugs or surgery may prevent Barrett esophagus and cancer (88), and surveillance of those with Barrett esophagus may reduce esophageal cancer mortality (93,94).…”

“…Since EAC is frequently detected at an advanced stage, the prognosis of patients with advanced EAC is still poor, with a 5-year overall survival rate of 10-20% [1,2]. Barrett's esophagus is a premalignant condition of the distal part of the esophagus in which normal squamous epithelium is replaced by columnar-lined epithelium with characteristic goblet cells [3].…”
