Survival of Proper Hepatic Artery Lymph Node Metastasis in Patients with Gastric Cancer: Implications for D2 Lymphadenectomy

Abstract: Background and AimsThere is a discrepancy between the American Joint Committee on Cancer (AJCC) guidelines (7th edition) and the Japanese treatment guidelines (3rd edition) with regard to the extent of D2 lymphadenectomy for gastric cancer. In the AJCC, hepatic artery station (No.12a) lymph node (LN) metastasis is classified as distant metastasis, whereas in the Japanese guidelines, this classified is regional metastasis. This study aimed to evaluate whether it is appropriate to reclassify No.12a LN metastasis… Show more

“…12a lymph node metastasis was correlated with the histological grade, AJCC stage, T stage, lymph node metastasis, intravascular cancer emboli, nerve invasion and tumor size of the patients with gastric cancer. These results were in agreement with those of Shirong et al ( 14 ). In the present study, the rate of metastasis for the No.…”

“…D2 lymphadenectomy has become globally accepted as the standard surgical procedure for curable AGC, with increasing studies of long-term oncological outcomes in eastern and western countries [20][21][22]. However, whether CME can contribute to LN dissection and benefit patients with AGC remains uncertain.…”

“…[11] Although the 7th AJCC classification was known to provide more accurate discrimination of patient survival or disease-free survival than the 6th edition, [1214] some critics had proposed that it was inappropriate to classify HDLN metastasis as distant metastasis. [15,16] A previous study, which was done in Korea, enrolled 1872 patients with gastric cancer having gastrectomy and D2 dissection with removal and pathological assessment of HDLN. This study demonstrated that the survival of patients with stages I to III disease with HDLN metastasis was better than that of patients with stage IV disease without HDLN metastasis.…”
