Survival of Transcatheter Mitral Valve Repair Compared With Surgical and Conservative Treatment in High-Surgical-Risk Patients

Abstract: Despite a higher log EuroSCORE, high-surgical-risk patients with symptomatic severe MR treated with transcatheter MV repair show similar survival rates compared with surgically treated patients, with both displaying survival benefit compared with conservative treatment.

“…Outside the EVEREST Randomized Trial, characterized by young low-risk operable patients, few studies have already compared transcatheter mitral repair with the MitraClip System to conventional surgery in the real-world setting, 9,10 focusing mainly on younger patients with functional MR and left ventricular dysfunction.…”

“…Moreover, even though most of these patients had a very high surgical risk, the procedure is associated with low in hospital complications [15,25,26]. Likewise, in small retrospective series, a survival benefit has been shown compared to medical management [27,28]. However, despite this objective benefit there is still a substantial population showing poor functional or clinical improvement.…”

“…The EVEREST II randomized trial compared the safety and efficacy of the MitraClip device to mitral valve surgery in low-risk surgical candidates and demonstrated slightly lower efficacy but lower complication rates [3]. Moreover, retrospective studies suggest better outcomes after PMVR in high-risk patients compared to medical therapy [4].…”
